Ah, if I force it to go via Oxford I get a Chiltern option. Is a ticket from Marylebone the same ticket as I'd usually get from Paddington? I wouldn't want to actually travel via Chiltern because it's a lot slower. Travel would be off-peak this Friday afternoon, returning on Saturday.

Two points here: the first is that the South Circular isn't such a major artery as the North Circular, it's more like just another road among several alternatives. But its named status might mean that it's more acceptable to mess around with it - removing on-street parking for example.

I think there would certainly be demand for it. The 472 bus from Thamesmead to North Greenwich was first envisaged as a tram and it's very busy (in normal times). A tram could resurrect the original route along the Woolwich Road or it could branch from Eltham as an elevated line above the A2...
Orbital travel is pretty difficult in South East London. Compare the North and South Circular roads to get an idea of how much suppressed demand there might be. There is also a lot of travel between this part of London and Kent, which is fine by public transport if you're on a convenient rail...

In the absence of any existing solution to that problem, I'd suggest troughs at the sump of the tunnel and a scoop on each train. Other than that, the drainage arrangements would be no different.
A tunnel would be more resilient and probably have a longer lifespan. A new approach would be needed anyway so it could reach the required depth without too steep a gradient. The evacuation system for rail is tried and tested in the Channel Tunnel.
But a long tunnel doesn't really work for road...
