Whatever the destination prints on the ticket normally, just type into the machine.
That BRFares website shows quite a few "MRAIL" areas/zones, so I'd type MRAIL into the machine...
If the ticket is discounted for being a wheelchair user, she is not entitlted to a seat and must remain in the wheelchair. If the ticket is full fare - she can get assistance on/off and sit anywhere.
That card will mean absolutely nothing in the UK.
Make a (free) seat reservation at least the day before - if possible.
Otherwise, if you don't have an obvious disability, it is unlikely that anyone will give their seat up for you sadly.

Worksop to Plymouth has two routes:
NOT VIA LONDON
+ANY PERMITTED
If you buy the +ANY PERMITTED ticket you can go via London (or on XC) in both directions.
If you buy the NOT VIA LONDON ticket, you will need a change of route excess for the outward portion.

I have had to give my details for quite a few 7 day season tickets. They scan the photocard barcode and my details appear. I sometimes get given a receipt with my name, house number and postcode on it to check.
Are you saying the booking office staff took your details or that one of the barrier/revenue staff did?
Both do have the powers to report you for the criminal offence of travelling without a valid ticket, so doesn't really make much difference BUT -
